Our assisted living home in Eaton Rapids helps seniors receive assistance with activities of daily living beyond what family caregivers are able to provide. With a higher caregiver-to-resident ratio compared to a typical nursing home in the area, we ensure attentively and specialized care to meet each resident’s specific needs. 24-hour assistance by dedicated and experienced caregivers, Our caregivers are trained to serve residents at our Residential Assisted Living & Memory Care Homes with Alzheimer’s and related dementia care needs.

Provides comfort, in a just-like home environment that gives a feeling of familiarity while providing personal space and privacy. Residents and their families are encouraged to personalize the residents’ space as much as they want. They are given the option to bring their favorite chair, blankets or pillows, and bed depending on the bed and room choice. Favorite family pictures such as wedding photos, siblings, photos of children and grandchildren as well as favorite paintings and books can help residents to feel more at home because they recognize these personal items and it reinforces their identity. Country Acres Home offers common areas that are decorated with modern furniture that provides comfort for residents as well as their families and guests. Modern furnished home Clean airy furnished rooms. Private room Personal items such as linens and towels, Patio space, Cable, Phone & Internet, Safety grab bars in bathrooms, Good natural and artificial lighting.

Each resident receives care based on their own personalized needs which may include: Eating assistance, Bathing assistance, Dressing and clothing selection, Personal hygiene, and grooming Adjustable and evolving care levels, Walking/transferring and transferring assistance Continence management Beauty services provided by a licensed beautician (Optional) In-house Home Health Care visits as needed for PT, OT, Speech or Nursing Care In-house medical assessments as needed by nurse practitioner under physician direction, Unlimited fruits and other snacks in between meals and as needed.

Not only is it a healthier option but eating together increases socialization and emotional well-being among our residents. During mealtimes, our professional caregivers can also support the hydration and food consumption of residents ensuring their physical well-being. We also celebrate birthdays and holidays with our residents.

Respite Care

We know that caring for an elderly person can be very challenging. It is even more demanding if the person is a senior citizen suffering from an illness such as Alzheimer’s or other forms of dementia. Country Acres Care Home provides a world-class respite care program designed for caregivers that need to get away, unwind, and refresh. Our guests experience care in a safe and secure community environment while receiving the needed medical care, engagement, and social stimulation they require. Caregivers will feel at peace knowing their parents or loved ones are in excellent hands with our highly trained, professional, compassionate staff.

Age in Place with Hospice

When Resident reaches a stage where medicine no longer helps and the level of care increases and care from a skilled nursing facility or nursing home cannot make things better, they do not have to worry about leaving the facility that has been their home. Country Acres will allow Hospice to come in and give the proper level of care needed to keep resident comfortable while still residing in the facility. The nurse will provide information to the family and caregivers as well as emotional support and counseling to the patient and their loved ones. They will order medication and give updates to the Doctor and require changes as needed. They are available 24 hours on Call and prevent the resident from using the 911 system and ambulance service to constantly go to the hospital which can cause a strain on financial resources. Hospice is covered by Medicare and most insurance also covers Hospice where the Nurse comes in and care for the resident on a frequency ordered by the doctor. Often times an Aide will also come in to assist the resident with bathing, grooming, and dressing. The most important role of Hospice is to continue the care of the resident and keep them as comfortable as possible while still remaining in the facility allowing them to keep their dignity as they progress in end of life.
